Consultations

A one-on-one meeting with a professional Care Manager at our office or in your home who will focus on your current concerns and issues. Following this meeting you will receive a written summary of recommended services and referrals prepared just for you.

Care Management

An on-going service that partners you with a dedicated Care Manager who will get to know you and put together a list of specific services and resources for you to select. This “Care Plan” is a guide for your short term and long term planning needs. Your Care Manager will support you in healthy aging through home visits and phone calls to ensure you receive quality services.

Health Care Coordination

A Care Manager or Senior Friendly Visitor will accompany you to medical and dental appointments & procedures as well as take notes, ask questions, schedule follow-up appointments, and keep family members and your health care professionals updated.

Education

The team at Lifematters Care Management embraces the philosophy that knowledge is power. We provide informative presentations on aging related issues at senior housing communities, religious establishments, Village groups and work place settings. We also provide Continuing Education presentations for Social Workers and Nurses on current topics in aging.

Lifematters has been an amazing resource for our family. When my father had a stroke, I reached out to them to help me with my parents, who were overwhelmed by how to deal with this life changing moment for them. I live on the other coast and my brother is 3 hours away with a busy job and family, so it was going to be hard for us to help on a daily basis. There was so much information and much confusion and lack of coordination between doctors, the hospital and the rehab facility. Lifematters took the time to meet with my family at the rehabilitation facility on a weekend, actively listened to what we needed and paired us with someone who has been an incredible match for our family. Currently, Medicare and Medicaid do not cover the cost of Care Management services, nor do most insurance policies. Some Long Term Care Policies may reimburse you a portion of your service. We are happy to review your policy with you to assess if Lifematters Care Management services are covered.

Lifematters billing procedures can vary based on services provided. A Lifematters representative will clarify billing procedures and options for you, including when payment is due to the company.

There is no average cost for home care and care management services because each situation is unique. Lifematters has various levels of professionals; with different levels of fees. The best suggestion is to contact us to discuss your unique situation.

Types of Home Care – Hours and Costs – Employee-Based

Hourly: Caregivers come for a set amount of hours based on the client’s needs and wants. In the DC metro area, the minimum is usually 4 hours and the average cost ranges between $28.00 and $33.00 per hour, plus mileage reimbursement. A caregiver hired for an overnight shift must remain awake and alert.

Live-in: A caregiver lives with his/her client. Caregivers need their own room to sleep and must have the ability to take breaks. The caregiver is allowed to sleep 6 to 8 hours overnight. The average cost for live-in care in the DC metro area ranges between $400.00 – $450.00 per day.

Transportation as part of your care program is available. Our client services directors will inquire if you need a caregiver who has their own vehicle or if you prefer to use your own vehicle. Whether you require assistance in getting to a doctor’s appointment, shopping, or if you just want a ride to visit a friend, we can help.

Lifematters offers a full range of private duty nursing services provided by licensed and trained LPNs (licensed practical nurses) and RNs (registered nurses). All skilled care cases are supervised by Lifematters RNs.

There is no right answer to this question. We are able to step in and assist you at any time. We can start when you think you require a more supportive living environment, during the loss of a family member, when you are in the hospital, when you need home care services or after a medical diagnosis.
