Care Management Services

Lifematters Care Management utilizes a team approach. Our nurses, social workers and other professionals work together to meet your needs. Our services are broad in nature but individualized to meet your needs.

Consultation

A one-time meeting with a professional Care Manager in your home who will focus on your current concerns and issues. Following this meeting you will receive a written summary of recommended services and referrals prepared just for you.

Care Management

An on-going service that partners you with a dedicated Care Manager who will get to know you and put together a list of specific services and resources for you to select. This “Care Plan” is a guide for your short term and long term planning needs. Your Care Manager will support you in healthy aging through home visits and phone calls to ensure you receive quality services.

Health Care Coordination

An Associate Care Manager will accompany you to medical and dental appointments & procedures as well as take notes, ask questions, schedule follow-up appointments, and keep family members and your health care professionals updated.

Quality of Life Associate

Someone to accompany you on the outings you enjoy such as seeing a show, visiting a museum, dining in a restaurant, shopping, and attending a lecture or event. We help you stay active by getting out and doing the things you enjoy.

Therapeutic Counseling

Lifematters Care Management is founded on the belief that individuals have the innate ability to change and self-actualize. We work with older adults in individual, couples, group and family sessions in a respectful and productive manner. Our therapists share a belief that being trusted with your challenges and goals is a privilege.

Nutrition

Nutrition is an integral part of any wellness plan. It can affect energy level, general health, sleep, mood and weight. We believe in a balanced and personalized approach to nutritional health incorporated with other healthy lifestyle choices. Lifematters Care Management provides counseling in cardiovascular nutrition, diabetes management and healthy eating programs.

Education

The team at Lifematters Care Management embraces the philosophy that knowledge is power. We provide informative presentations on aging related issues at senior housing communities, religious establishments, Village groups and work place settings.
